Steps to reproduce:

1. Observe ezimagefile table.
2. Create content of "Image" Content Type and add a single image to it, publish it.
3. Edit and publish again without changing any data.

Result:
There are 3 records in ezimagefile table, they differ only by id column.

Expected result:
There should be a single record corresponding to filepath and contentobject_attribute_id columns.
