Add validation for duplicated Taxonomy Entry Assignment fields.

Using multiple Taxonomy Entry Assignment fields for the same taxonomy, as discovered in IBX-3773, causes issues in the admin panel.

Validation should be added to cover this case, ideally informing about recommended solution according to feature design.
