    Description

      The page http://ez.no/doc/ez_publish/upgrading/upgrading_to_4_0/from_4_0_x_to_4_0_y doesn't appear to have been updated since 4.0.1 came out, and is missing information from the 4.0.2 release notes at http://ez.no/developer/news/ez_publish_4_0_2_released that is very useful when upgrading.

      It would perhaps be useful to mention the information on the http://ez.no/doc/ez_publish/upgrading/upgrading_to_4_0/from_3_10_x_to_4_0_y page too, since if you upgrade from 3.10.1 straight to 4.0.3 you miss the instructions which create the ezurlalias_ml_incr table, and get fatal errors when trying to publish on the new site.
