This might be related to EZP-27151 , the removal of content draft, where an image field has been updated, does not delete the image folder and file.

steps to reproduce

• create a content type with two fields: Text Line and Image
• create and publish content of this type
• create a new version, change the image, save but do not publish
• remove the draft (using versions tab in the back end)
  => the folder and file for the new image is kept in var folder

(you can also create and not publish several new versions and then remove them all using the same feature and you'll be left with a lot of garbage in the disk)
